Our Care Coordination for Families & Individuals in Crisis service provides a clear, structured pathway for stabilizing difficult situations and guiding you toward the right resources, decisions, and next steps.
What Is Care Coordination?
Care Coordination is a personalized support service designed to help individuals and families manage complex emotional, relational, or situational challenges. You receive a dedicated coordinator who helps you:
- Bring clarity to chaotic or overwhelming situations
- Communicate effectively across providers, caregivers, or family members
- Identify the right resources at the right time
- Create a safety‑focused plan to stabilize the environment
- Break problems into manageable steps
- Ensure consistent follow‑through so nothing falls through the cracks
You don’t have to navigate crisis alone.
Who This Service Helps
Care Coordination is ideal for:
- Families in active conflict or navigating high-stress transitions
- Parents overwhelmed by behavioral, emotional, or school-related challenges
- Individuals managing trauma, burnout, or emotional distress
- People dealing with medical/mental health systems and needing clarity
- Anyone who feels stuck, unsupported, or unsure of what to do next
Whether the crisis is emotional, relational, medical, or situational—support is available.
What You Receive
Every Care Coordination client receives:
1. A Dedicated Point of Contact
Your coordinator stays with you throughout the entire process, helping you make sense of what’s happening and what comes next.
2. Stabilization Plan
We map out immediate needs, risks, and steps to restore safety and clarity.
3. Resource & Provider Navigation
We help you identify and connect with the right therapists, medical providers, legal resources, community programs, and supports.
4. Communication Assistance
We support communication across systems—so you’re no longer stuck managing everything alone.
5. Follow‑Through
Your coordinator stays connected to ensure progress continues and nothing is missed.
Limited Openings for the Holiday Season
To ensure every individual and family receives the depth of care they deserve, Care Coordination spots are extremely limited. A small number of openings are available during the holiday season.
If you may need support, early outreach is strongly encouraged.
You are not alone. Help is available.
How to Get Started
1. Submit a Request for Support using the button to the Google form below.
2. We complete a brief screening to understand your needs.
3. If your situation is a match for Care Coordination, you will be offered one of the limited openings.
4. Your coordinator begins support immediately.
